A member of the production crew for the upcoming film Resident Evil: The Final Chapter died from injuries sustained during an on-set accident in South Africa earlier this month.

Entertainment Weekly reports that 34-year-old Ricardo Corenlius died December 3 in what German production company Constantin Film called a “tragic accident.”

From the EW report:

“Ricardo Cornelius died on Dec. 3 after an accident during a prep day involving safety checks on the Cape Town set of the film, a statement from the German production company responsible for the film [says]. According to Constantin Film, witnesses say he was helping to rotate a Hummer on a turntable during a rehearsal when he slipped and became trapped between it and a wall.”

This accident did not happen during filming, which wrapped up on December 9. EW reports that an investigation into the accident may be ongoing.

“We are all deeply shocked and saddened by this tragic accident, which was out of anyone’s control,” a spokesperson for Constantin said. “Our thoughts and prayers are with his family.”

This is not the first accident to happen on the Final Chapter set. Earlier this year, Milla Jovovich’s stuntwoman, Olivia Jackson, sustained serious injuries while filming an action sequence. Her left arm is paralyzed and will need to be amputated.

The Final Chapter comes to theaters in January 2017.
